– Tom Burns is Professor of Social Psychiatry at the University of Oxford and currently visiting professor at Gothenburg. CMHTs development paralleled deinstitutionalisation from the start of the 1960s. France and the UK led the movement which was pragmatic and clinically led. In the era of evidence based practice since the 1980s CMHTs there has been intense research investigation into them, usually as ACT or Case Management services.

Contradictory results in much of this 'black-box' research has led to a clearer understanding of the central ingredients of good care and a recognition of considerable redundancy in much of the model description.

These core ingredients will be outlined and their limitations (e.g. a failure to reduce coercion) acknowledged.
